I offer a flat-rate ADHD evaluations for children (ages 4 and up) or adults. These evaluations are more streamlined and are designed for children or adults who may have attention problems.

Who might benefit from an ADHD evaluation:

• A child who:
  • is easily distracted
  • has received feedback from their teacher that they have difficulty attending in class
  • forgets about homework
  • makes careless errors
  • has difficulty with organization
  • has difficulty doing work
  • has difficulty completing tasks
  • has difficulty sitting still
• An adult who:
  • is easily distracted
  • has difficulty finishing things
  • loses things
  • is forgetful
  • has difficulty paying attention to other people when they are talking
  • interrupts people
• This streamlined assessment may not be appropriate for people who are:
  • wondering if they have a learning disorder in addition to attention problems
  • actively using substances

Process:

• Email or call to set up an initial appointment.
• Schedule a 2- to 3-hour appointment.
• During the appointment, I will ask your specific questions about your life/your child’s life at home and at school/work. It may be helpful if you bring any specific feedback you have received about your performance at school or work. During the appointment, I may also ask you to complete some short assessment measures. I will also ask you to complete some short online questionnaires. I may also ask your permission to send a similar questionnaire to someone who knows you well.
• After the appointment, I will prepare a report summarizing the interview and assessment measures.
• The flat-rate also includes a feedback session which can be completed online or in-person. During the feedback session, we will review the report and any recommendations.

Fees:

• I offer a flat-fee for ADHD-only assessments. This includes the interview and testing appointment, completion of report, and feedback meeting. Contact me for details.
• I am also in-network with Blue Cross Blue Shield and can bill the insurance company directly.
• A deposit will be due at the end of the first meeting and a final payment will be due on the last meeting. I accept cash, credit card, and check.
